Understanding the SOURCE Program
The SOURCE Program (Service Options Using Resources in a Community Environment) is a Georgia Medicaid program designed to help eligible seniors and adults with disabilities receive healthcare and support services while remaining in their homes and communities. The program combines primary medical care with home and community-based services to improve quality of life and reduce unnecessary hospitalizations.

✔ Age 65 or Older OR Adult with a Disability
✔ Requires Nursing Home Level of Care
✔ Able to Safely Live at Home with Support Services
